    • We need to obtain personal details including name, date of birth, health issues and medication information in order to ensure we can safely carry out treatments. Some medication and health issues are a contra-indication to a treatment, meaning a service may need to be adapted as necessary or cannot be performed. It is vital that we are kept up to date of any changes to your medical records to ensure your safety. We also need to keep records of patch testing results, this is a legal requirement. If this information is not provided, treatment will be refused as I cannot guarantee your health & safety without this information.
